Leave of Absence Request Form

A leave of absence is an extended period of time away from your job. Reasons, why an employee would request a leave, might be: maternity leave, sick leave, jury duty and personal/family. The process requires the LOA form to be completed as it documents the reason for the absence and along with having a record on file.
